The Application

The SMDEP Web application allows you to register with the National Program Office and obtain a username and password. Upon successful registration, the applicants will be assigned an SMDEP I.D. number, which can be viewed in the online or printed application. Applicants must certify and submit the SMDEP application to enable their designated site(s) to view their application data. The successful certification and submission of the application and the receipt of all transcripts and letters of recommendations by the National Program Office will instantly create a full record for the sites to review. The application will not be considered complete until all documents are received.

Commuter Program Supplemental Application

This year six SMDEP program sites are offering applicants an opportunity to participate in the program without living on campus. SMDEP historically has a residential component required for participation. The Robert Wood Johnson Foundation recognizes that students have commitments that would preclude them from participating in the residential program, and prevent them from attending SMDEP. Students interested in living off campus and attending one of the participating sites must complete the 2012 Commuter Application Form in addition to the SMDEP Web Application. The following sites are offering a non-residential/commuter component:

Case Western Reserve University

Columbia University

Duke University

University of Texas-Houston

UMDNJ-New Jersey Medical and New Jersey Dental

University of Washington

Acceptance Procedures

SMDEP program sites are completely autonomous in making admission decisions. The SMDEP National Program Office has established guidelines regarding acceptance procedures for sites and applicants. SMDEP program sites are expected to follow these guidelines.

Dates and Deadlines

For the Summer 2012 Session:

Application opens: November 1, 2011 at 12:01 a.m. EST

Application closes: March 1, 2012 at 5:00 p.m. EST
