The information on the front of the SMDEP Recommendation Form
is important for processing. If you use a letter, please fill
out the front of the SMDEP Recommendation Form and attach it
to the letter. Note: the letter must be on official
institution letterhead.

What is a prehealth advisor?
May be a faculty or staff member at your institution in
your dean's office, career or student services center.

How can a prehealth advisor assist me?
A prehealth advisor can help you identify premedical or
predental course requirements, help you seek enrichment programs
and clinical experiences, prepare for application to medical
or dental school, write letters of recommendation, and assess
the most appropriate career path for you.
If you are unable to identify a prehealth advisor at your institution,
contact the National Association of Advisors for the Health Professions

Can the form be completed by a previous employer or high
school teacher?
SMDEP programs accept recommendations from employers in the
medical field and from high school teachers in the sciences.
You will be contacted if further recommendations are needed.
Be sure to complete the front of the SMDEP Recommendation Form
for accurate processing.

Can a recommendation be written for me by a teaching assistant?
If your teaching assistant is acting as your instructor and
knows you well, SMDEP programs will accept their recommendation.

May I submit more than two recommendations?
The SMDEP will accept no more than two letters of recommendation.
